Publicación Restringido Abordaje del TDAH desde el diagnóstico hasta el tratamiento en niños y adolescentes: Una revisión bibliográfica(Universidad Católica de Pereira, 2017-07-15) Galvis Palacio, Juan Carlos; Arenas, José IgnacioEn el presente trabajo se ha realizado una revisión sistemática de la literatura científica con la finalidad de evidenciar el itinerario protocolar más usado para el abordaje del déficit de atención/hiperactividad desde el diagnóstico hasta el tratamiento. Para alcanzar el objetivo se pretende describir las variables epidemiológicas, patogénicas, los factores de riesgo y de pronóstico, las comorbilidades y las herramientas de evaluación. Se ha encontrado que el diagnóstico del déficit de atención/hiperactividad sigue siendo un proceso complejo, por ende, es necesario tener en cuenta el desempeño del niño en diferentes contextos sociales, prevalentemente el familiar y el escolar, además es necesario discriminar cada uno de los comportamientos y contrastarlos con los criterios descritos en el DSM-5. El tratamiento más usado, a pesar de sus efectos adversos y de generar ventajas solamente a corto plazo es el farmacológico con el uso de psicoestimulantes. Sin embargo, las intervenciones psicosociales pueden alcanzar resultados positivos en aquellas áreas de desempeño (cognitivo, social y emocional) no alcanzadas por la intervención farmacológica, ya que un 30% de los niños y adolescentes no reaccionan bien a los fármacos y más de un 25% de los padres no adhieren al tratamiento a causa de las adversas consecuencias que producen los medicamentos a la hora de ser usados por periodos prolongados.\ Abstract: In the present work, a systematic review of the scientific literature has been done with the purpose of evidencing the protocolary route most used for the attention deficit / hyperactivity approach from diagnosis to treatment. To reach the objective, we intend to describe the epidemiological, pathogenic variables, the risk and prognostic factors, the comorbidities and the evaluation tools. It has been found that the diagnosis of attention deficit / hyperactivity remains a complex process, therefore, it is necessary to take into account the child's performance in different social contexts, predominantly the family and the school, in addition it is necessary to discriminate each one of the Behaviors and contrast them with the criteria described in DSM-5. The most widely used treatment, despite its adverse effects and generating benefits only in the short term, is the pharmacological treatment with the use of psychostimulants. However, psychosocial interventions can achieve positive results in those areas of performance (cognitive, social and emotional) not reached by pharmacological intervention, since 30% of children and adolescents do not react well to drugs and more than 25 % Of parents do not adhere to treatment because of the adverse drug consequences of being used for prolonged periods.Publicación Restringido La autorregulación en el TDAH: una visión desde una perspectiva neuropsicológica(Universidad Católica de Pereira, 2016-04-03) Solarte Bastidas, Mónica Elizabeth; Castaño Henao, Diana CarolinaEn este artículo se presenta una revisión sobre las intervenciones de la autorregulación en los niños con TDAH. Se exploran las propuestas de tratamiento más utilizadas desde los abordajes farmacológicos y no farmacológicos, haciendo énfasis en el tratamiento neuropsicológico de los déficits de la memoria de trabajo que inciden en el proceso de autocontrol del pensamiento y la conducta. Se presentan estudios que avalan y cuestionan la eficacia de estas intervenciones con el objetivo de proponer una nueva forma de abordaje de la autorregulación basada en la estimulación de la memoria a largo plazo por medio del repaso espaciado (RE). La técnica de RE puede instaurar repertorios conductuales de autorregulación más funcionales y automatizados, permitiendo sentar las bases para un tratamiento más viable, ecológico y con un bajo impacto económico para las instituciones de salud pública y los padres de niños con TDAH./Abstract: This article is a review on the intervention strategies of self-regulation in children with ADHD. It explores the most commonly, both pharmacological and non-pharmacological interventions, emphasizing the neuropsychological treatment of the deficits of working memory that affect the process of self-control of thought and behavior. In this work are analyzed studies that support and question the effectiveness of these interventions with the purpose to propose a new form to approach self regulation based on spaced retrieval (SR) technique. El usuario llega a consulta diagnosticado con TDAH con predominio hiperactivo e impulsivo. Esta investigación se realizó mediante metodología con enfoque cualitativo descriptivo desde un estudio de caso; con el abordaje cognitivo conductual. En cuanto a la obtención y análisis de la información, se hizo uso de estrategias de recolección de datos como lo son la historia clínica, entrevista, observación y registro; finalmente obteniendo como resultado el desconocimiento del diagnóstico por parte de los padres del paciente que llevaba consigo una crianza demandante y el no ajuste curricular por parte de la institución educativa. Abstract: The present case study was carried out with the objective of describing the behavior of a 6-year-old patient, enrolled in second grade, treated at the Psychological Care Center (CAPSI) of the Catholic University of Pereira, since he works with the consultant in a postgraduate internship context. The user comes to the consultation diagnosed with ADHD with a predominance of hyperactivity and impulsiveness. This research was carried out using a methodology with a qualitative descriptive approach from a case study; with the cognitive behavioral approach. That's why is necessary to make an analysis of the main signs causes and effects for determinate that are the different treatments more empiric efficient to control the problematic. In this article the main treatments, such as cognitive-behavioral and pharmacological multimodal contribute to the sings reduction. It is also important to mention that the ADHD is a neurodevelopmental problem that affects in the areas of social interactions and academic developments of those who have it, therefore, it can be demonstrated that drug treatment is not sufficient and therefore do necessary cognitive-behavioral and multimodal interventions. Se realiza un recorrido histórico a lo largo de los años, abarcando la tipología, sus principales síntomas y cómo se afectan ciertas funciones ejecutivas y cognitivas. El texto trata de establecer los principales modos de tratamiento, los medicamentos usados para combatir este padecimiento, al igual que algunos tratamientos alternativos, que carecen de posología. Además expone que las personas más cercanas al pequeño son los principales encargados de proporcionar información para brindar un diagnóstico oportuno, y evitar la omisión en casos graves, pues un tratamiento adecuado puede permitir una relativa normalidad cognitiva, así como un mal diagnóstico puede afectar profundamente el desarrollo del infante.\ Abstract: The following compilation seeks to highlight the ADHD (Attention Deficit Hyperactivity Disorder) as one of the problems of social and cognitive development afflicting the population, especially children, emphasizing on children of 7-10 years. A historical tour along the years, covering the typology, the main symptoms and how certain executive and cognitive functions are affected. The text seeks to establish the main treatments, the drugs used to treat this disease, as well as some alternative treatments lacking dosage. Se describen los abordajes farmacológicos, cognitivo-conductuales, multicontextuales y multicomponentes. Estos últimos representan una proyección hacia el futuro en las intervenciones en el TDAH. Aunque se encuentren limitaciones, como por ejemplo, la poca implementación del tratamiento dirigida a los padres y a los maestros, se considera que futuras investigaciones puedan incluir estas variables y conferir mayor objetividad a este tipo de intervención.\ Abstract: In this reflection paper we make a presentation about the nature, pathogenesis and comorbidity of attention deficit hyperactivity disorder (ADHD), emphasizing the types of treatment. Pharmacological, cognitive-behavioral, multicontextuales and multicomponent approaches are described. These last ones represent a projection into the future interventions in ADHD. Él se concentra y permanece quieto mediante la estimulación de sus sentidos, porque este tipo de motivación hace que se sienta más seguro de sí mismo. The child cannot stay to long doing the same activity because it gets distracted easily; the child concentrates and sits still through the stimulation of his or her senses because it makes them feel surer of themselves. Donde se establece de manera general y no especifica en niños, que la empatía como habilidad juega un papel fundamental en la eficiencia y la determinación de la relación terapéutica, entre terapeuta – consultante, así mismo se resalta que la relación se construye por ambas partes y que el terapeuta debe ser flexible y generar una adecuada adherencia al tratamiento para lograr resultados exitosos y prolongados. Se han descrito las principales pruebas de evaluación neuropsicológicas aplicadas a niños que presentan síntomas de TDAH en edad escolar, para reconocer sus principales condiciones a nivel de: funciones ejecutivas, de organización y planificación, académicas, signos neurológicos, emocionales, memoria, rendimiento académico y neurocognitivo, familiar, coeficiente intelectual y auto-concepto.\ Abstract: The present article is a scientific compendium about attention deficit hyperactivity disorder (ADHD), based on Forty-four references from Chile, Spain, Colombia, Argentina, Mexico and USA with psychometrical, neuropsychological and behavioral approach. Para tal fin, se realizó un estudio de tipo cuasiexperimental basado en un estudio de caso único, en donde se elaboró una conceptualización de las variables implicadas, como también se hizo una prueba pre y post test de la educación musical con el objetivo de medir los procesos atencionales. Los resultados muestran una mejora en las puntuaciones del postest, fundamentalmente en los procesos de atención. Abstract: this research sought to determine what is the incidence of music education in the attentional processes of a child suffering from attention deficit hyperactivity disorder and mild intellectual development disorder. To this end, a quasi-experimental study was carried out based on a single case study, where a conceptualization of the variables involved was elaborated, as well as a pre and post test of music education with the objective of measuring the attentional processes. El presente artículo de reflexión efectúa un análisis de los componentes de dicha patología; causas, epidemiología, comorbilidades, alteraciones cognitivo-conductuales intervenciones farmacológicas y no farmacológicas, con el objetivo de exponer la musicoterapia como tratamiento alternativo y coadyuvante en el tratamiento psicoterapéutico, sobre el cual existen acercamientos científicos y sistemáticos que sustentan la viabilidad de su aplicación en Niños, Niñas y Adolescentes (NNA) para la reducción de los síntomas del TDAH. El documento concluye que la intervención farmacológica debe realizarse de manera adicional a las terapias combinadas, conductuales, comunitarias y musicales. Igualmente se concluye que, aunque se hace necesario efectuar investigaciones rigurosas que validen el procedimiento, la información empírica indica que la musicoterapia es coadyuvante en el tratamiento psicoterapéutico, contribuyendo en el desarrollo cognitivo y de autoestima del sujeto intervenido.\ Abstract: Attention Deficit Disorder with Hyperactivity (ADHD) is classified as mental pathology of neurobiological origin whose main characteristics are: inattention, hyperactivity and impulsivity. The present article of reflection carries out an analysis of the components of said pathology; causes, epidemiology, comorbidities, cognitive-behavioral alterations pharmacological and non-pharmacological interventions, with the aim of exposing music therapy as an alternative treatment, on which there are scientific and systematic approaches that support the viability of its application in children and adolescents for the reduction of ADHD symptoms. The paper concludes that pharmacological intervention should be performed in addition to combined, behavioral, community and musical therapies. El foco de observación se centra en el funcionamiento del sistema familiar, su interacción con el medio escolar y el impacto en la dinámica relacional del diagnóstico el cual emerge del medio externo desequilibrando la familia al interior. Se llevó a cabo una búsqueda de artículos publicados entre el 2010 y 2021 por medio de bases de datos como Pubmed, Scopus, Scielo, Redalyc, BVS y Science Direct. Siguiendo el protocolo Prisma, los términos de búsqueda utilizados fueron: autoconcepto, autoestima, autoimagen, autopercepción, TDAH, niños, adolescentes y las mismas palabras en inglés. Fue posible identificar una tendencia hacia el autoconcepto negativo y no se encontraron resultados concluyentes respecto a la estabilidad o cambio de este con respecto a la edad. Finalmente, se reconoce la importancia de una comprensión integral del trastorno que involucre un diagnóstico de áreas tanto a nivel cognitivo, como comportamental y psicológico. Abstract: The aim of this systematic review was to analyze self-concept in children and adolescents diagnosed with ADHD. A search of articles published between 2010 and 2021 was carried out using databases such as Pubmed, Scopus, Scielo, Redalyc, BVS and Science Direct. Following the Prisma protocol, the search terms used were: self-concept, self-esteem, self-image, self-perception, ADHD, children, adolescents and the same words in English. It was possible to identify a tendency towards negative self-concept and no conclusive results were found regarding its stability or change with respect to age. Finally, the importance of a comprehensive understanding of the disorder involving a diagnosis of cognitive, behavioral and psychological areas is recognized.
